OFFICE VISITS

Newborn Hospital Follow Up

Welcome and Congratulations on your new baby!

If your baby will be born at Sarasota Memorial Hospital (SMH, Main Campus), please name us as your baby's pediatrician.  One of our doctors or nurse practitioners will see your baby while you are admitted and we will call you set up your first follow-up office visit.

If your baby will be delivered at a different hospital, please call us on the day that you are discharged from the hospital so that we can set up your first follow-up visit with us in the next 1-2 days.

For your follow-up visit, please arrive 15 minutes prior to your scheduled appointment and bring your newborn's insurance card.

Well Visits

Well Visits (also known as yearly/annual physical exams) are a time to see check in to see how your child is growing, address concerns and answer questions, and make sure your child stays on track with recommended screenings and immunizations.

​At these visits, we can also complete physical forms for sports, camps, etc.  Please be sure to bring any necessary forms to your child's well visit appointment.

Your Child's Checkups 

Sick Visits

If your child is sick, give us a call and we will do our best to fit you in to see a medical provider the same day.  We also offer a walk-in clinic for established patients on weekday mornings from 8:00 - 8:30 am (first come, first served).

Other Visits

Medication Consults

Usually every 3 months for patients taking medication for Mental or Behavioral Health.

Pre-Operative/Pre-Procedure Clearance

Medical clearance for surgery or outpatient procedures. Please bring all necessary forms to this visit.

Hospital Follow Up 

Visit after any hospitalization, emergency room visit, surgery, or procedure for continuity of care.

​​

Vaccine-Only Visit

Nurse Visit to receive routine or seasonal (Flu) vaccine(s).
